The theory is that a Hellmouth is a portal to hellish demon dimensions, not a portal to an 'actual' post-life Hellscape. Opening the Hellmouth is an Apocalyptic event because it would provide a continuous open portal to such a dimension, as opposed to the 'temporary' and 'limited' portals which can be opened through magic, or various abilities of some demon species (such as Vengeance Demons who can teleport in different dimensions).
Glory, for example, was the ruler of one such dimension, and intended to use the Key to open a portal to her dimension (which wasn't a post-life Hell for souls, but a different dimension inhabited by 'hellish' demons - same as Quor'Toth, where Connor was raised.)
There might be different such 'dimensional mouths' that connect to other types of dimensions as well. For example places such as ancient Delphi, or Stonehenge, might have been constructed in such 'blessed' locations. There the walls between Earth and other, 'heavenly' dimensions would have been weaker, allowing for more 'positive' energy to flow through. Such places wouldn't be appropriate for human inhabitation, because of all the sins that would corrupt (and weaken) the 'heavenmouth.' Therefore only temples/sacred places/burial grounds would be in such places, if they aren't located in remote locations such as mountaintops.
Hellmouths would, on the contrary, welcome both human and demon populations because the sins and crimes that would happen there would 'feed' the Hellmouth (in a sort of sacrificial way).
Actual 'heavenly' and 'hellish' planes of existence (post-life, such as the Heaven described by Buffy), would only be accessible to 'elevated' beings such as the PTB, not for 'traffic.' So Anya, or even possibly even D'Hoffryn, would not be able to teleport to such a plane of existence, despite their dimension-hopping abilities.
